Patient Portal

Please utilize the patient portal to contact us. It is the most efficient way to get ahold of us. The provider answers the phone between patients and there is not a receptionist on site. 

Patient requirements

  • You must have a valid email address that will be linked to the patient portal.  

  • ​You must have a valid debit or credit card on file for billing copays, deductibles, and coinsurance payments, due at the time of service.
